Sick notes

Ways to request a sick note:

-you can send us an email to nclicb@tottenhamhealthcentre@nhs.net providing the following details 

• Full Name 
• Date of Birth 
• How long you want the sick note for
• The reason for the sick note describing the illness 

- alternatively you can bring a paper request writing the details above  and hand it over to the reception desk.

Please note sick notes will take 1-3 working days to be proccessed 

If you’re off work sick for 7 days or less, your employer should not ask for medical evidence that you’ve been ill. Instead, they can ask you to confirm that you’ve been ill. You can do this by filling in a form yourself when you return to work. We call this self-certification.
