Repeat Prescriptions

red_orange_pillsRequests must be made in writing using the slip attached to your prescription.

If you do not know the name of your medication or if you lose the repeat prescription slip, please write a letter of request with your full name, DOB, telephone number and what this medication is for.

You can also attend in person in reception or send request by post. You may also place your pescription request in the letter box outside the practice.

Please do not send fax request as we do not get details correctly – Medication errors are serious.

Please do not ring Surgery to request it over the telephone.

Once received requests will be processed but no e-mail/phone confirmation of acknowledgement will be made.

Medication Reviews

Patients on repeat medication will be asked to see either a doctor, nurse practitioner or practice nurse at least once a year to review these regular medications.

Please allow two full working days between requesting and collecting your medication
